Successful Management of Chronic Fistulized Infection with Implant Placement and Guided Surgery

A patient presented with a chronic fistulized infection in the upper right central incisor, resulting from a horizontal fracture in the middle third of the tooth. After several months of infection, the treatment plan involved an extraction, curettage, and alveolar preservation using biomaterial, alongside a gingival punch procedure. A provisional restoration was placed using fiber, followed by the placement of a Cortex Dynamix Conical implant six months later.

Guided surgery and immediate loading were utilized to overcorrect the gingival contour. After four months, the gingival margin was refined using the provisional restoration, and the area was scanned for the definitive crown. The case was performed at Noxadent Dental Clinic in Madrid, Spain, with surgery and provisional restoration by Dr. Alfonso García and the definitive restoration placed by Dr. Pablo Gómez Cogolludo.

Successful Implant Placement and Guided Bone Regeneration for Fractured Tooth #15
Dr. Davis Fonseca, Mexico
A 58-year-old female with no significant medical history presented with pain and discomfort in the posterior right maxillary region, where clinical examination revealed a longitudinal fracture in tooth #15 and a periapical lesion on radiographs. The tooth was non-restorable due to the fracture and mobility. A treatment plan was developed, involving the extraction of tooth #15 and the placement of a dental implant. A CBCT scan confirmed bone availability and identified the need for guided bone regeneration due to a lack of buccal cortical bone.

The procedure began with atraumatic extraction using odontosection to preserve the bone structure, followed by implant placement (Cortex Dinamix 4.2×11.5 mm) at 40 Ncm torque.
Guided bone regeneration was performed using a collagen membrane and human bone graft, with plasma application to promote healing. After 4 months, a healing abutment was placed, and the definitive restoration, a porcelain-fused-to-metal crown, was fabricated to match the natural tooth anatomy. Four years later, radiographs confirmed excellent osseointegration, and the patient reported improved chewing function and satisfaction with the aesthetic outcome. This case highlights the importance of advanced imaging, careful planning, and precise execution in achieving successful, functional, and aesthetically pleasing implant rehabilitation.
